Use this form if you need to request a replacement R’Card (or badge) for one that was lost, stolen, or damaged.

As of September 11th, 2023 the new Gender Recognition and Lived Name Policy went into effect. If you have a lived name on R'Web please use that name when completing this form as the Card Office will only be able to verify that identity, and not your legal name.
